This Privacy Policy explains how Ground Up Rehab, PLLC collects, uses, discloses, and safeguards information obtained through the Site.

Automatically Collected Data – IP address, browser type, device identifiers, pages viewed, time and dates you visit the Site, and cookies. We also collect information you provide to us – name, e-mail, phone number (DO NOT INCLUDE MEDICAL DETAILS). .

Ground Up Rehab, PLLC uses third party service providers, including but not limited to Google Analytics and cookies (social media pixels and digital advertising pixels) in order to improve our service, user experience, and to analyze how the Site is used. Aside from your device’s Internet Protocol address (IP address), which can disclose your general location, the information collected by Google Analytics and Adobe Analytics is anonymized traffic data including length of stay on the Site, browser information, and device information. The collected information is used to provide an overview of how people are accessing and using the Site. 8. Security
We use reasonable administrative, physical, and technical safeguards to protect data. No system is 100 % secure; use the Site at your own risk. While we do our best to protect your personal information, we cannot guarantee the security of any information that you transmit to us and you are solely responsible for maintaining the secrecy of any passwords or other account information. It is your sole responsibility to promptly inform Ground Up Rehab, PLLC of any need to deactivate a password.
9. Data Retention
We retain PII and PHI for as long as necessary to fulfill the purposes described above and as required by law (typically 7 years for medical records in Illinois).
- Contact‑form data: deleted after 24 months of inactivity.
- Usage data: kept anonymously for analytics up to 24 months.
- PHI in PracticeQ, WebPT/Therabill, Dropbox, and/or Microsoft Cloud: retained per HIPAA and Illinois requirements (typically 7 years for adults, until age 23 for minors).
10. Your Rights
Depending on your jurisdiction, you may have the right to access, correct, delete, or restrict processing of your personal data. Submit requests to info@grounduprehab.com. Verification steps may be required.
11. Children’s Privacy (COPPA)
The Site is not directed to children under 13, and we do not knowingly collect personal information from anyone under 13. If we learn that we have received personal information from a child under 13 without verified parental consent, we will delete that information promptly. We comply with the Children’s Online Privacy Protection Act (COPPA) and applicable Illinois privacy laws regarding minors’ data.

Text Messaging (SMS) Terms & Opt-Out

- To stop receiving a specific type of text message, simply reply with the word “STOP.”
- You’ll receive a confirmation that you’ve been unsubscribed from that type of message.
- Other messages you’ve opted into may continue unless you opt out of those separately.
- To resume messages you’ve previously opted out of, re-enroll using the same method you originally used (e.g., intake form or registration).
If you need assistance with any text message service, reply “HELP” or contact us directly at info@grounduprehab.com. Standard message and data rates may apply, depending on your mobile plan. Ground Up Rehab, PLLC is not responsible for any charges you may incur. While most patients receive no more than 3–5 texts per week, frequency may vary based on your appointment schedule or participation in clinical follow-up programs. Carriers are not responsible for delayed or undelivered messages.
Email Terms & Unsubscribing
If you provide your email address, you may receive both clinical communications and optional promotional content from Ground Up Rehab, PLLC. By sharing your email, you confirm that you have access to a secure device or email account. You may unsubscribe from promotional messages at any time by clicking the “unsubscribe” link included in the footer of each email or by emailing info@grounduprehab.com.
13. Changes to This Policy
