Ground Up Rehab is a physical therapy business specializing in geriatric, orthopedic, and sports physical therapy, as well as fitness and sports performance training. Using technology-enabled tools and data-informed care, we deliver these services directly to patients’ homes through our mobile platform, at our Chicago clinic location(s), and via telehealth or consultations as needed.

Trainer & Founder

Davor Vasiljevic

Dr. Davor Vasiljevic is a board-certified orthopedic and sports clinical specialist with over a decade of experience in physical therapy, athletic performance, and clinical research. He holds dual certifications in Orthopedics (OCS) and Sports (SCS) from the American Board of Physical Therapy Specialties, and is also a Certified Strength and Conditioning Specialist (CSCS). He works with everyone from elite athletes to older adults regaining mobility and independence.

His expertise includes post-operative, geriatric, and neurological rehab, injury prevention, and performance optimization. With advanced training in dry needling, manual therapy, DNS, MSI, and over 25 years in martial arts, Dr. Vasiljevic creates personalized treatment plans to improve movement and speed recovery.

He has supported athletes at world-class events like the Chicago Marathon and IJF World Judo Championships, and worked at a top neurological rehab center. Holding a Master’s in Clinical Investigation and several publications, he bridges research with real-world care.

As founder of Ground Up Rehab, he brings expert therapy and coaching directly to patients’ homes through a mobile concierge model, also offering in-clinic sessions. His flexible approach supports busy professionals and older adults with specific mobility needs.

Do you offer performance programs or consultation services beyond rehab?

What makes Ground Up Rehab different from other physical therapy providers?

We deliver a concierge experience:

Do you accept insurance?

Yes. We are in-network with Medicare and Blue Cross Blue Shield of Illinois (BCBSIL).For all other insurance plans, we provide out-of-network billing support, including superbills and documentation to help you seek reimbursement. We may provide out-of-network submission on your behalf in some cases.

What areas do you serve for in-home visits?

We currently serve multiple neighborhoods throughout the greater Chicago area, including Downtown, the North Shore, as well as some Northwestern and Southwestern Suburbs. Please contact us to confirm availability and pricing in your area.

What should I expect during a home visit?

Your therapist will arrive on time with all the equipment needed—including a treatment table, resistance bands, modality tools, and more. We treat your space like a professional rehab setting. If specialized equipment is required, sessions may also be held in our clinic gym.

Do I need a referral to start therapy?

In most cases, no referral is needed to start physical therapy. Illinois law allows direct access with some limitations and most insurance plans do not require one. We are happy to confirm your insurance benefits and any prescription or pre-authorization requirements. 

Who will be my therapist?

You’ll be matched with a dedicated physical therapist based on your needs and availability. That therapist will remain your point of contact throughout care—managing evaluation, treatment, follow-up, and progress. This continuity is part of our concierge advantage.

Do you treat older adults or Medicare patients at home?

Absolutely. We specialize in geriatric physical therapy and are fully equipped to treat Medicare beneficiaries in the comfort of their own home with excellent results. In most cases, Medicare beneficiaries with a secondary policy can expect to pay nothing for physical therapy services. (Home Delivery Fees apply.)

How long are the sessions?

Most sessions last 45 to 60 minutes, depending on your condition and personalized goals. We don’t rush appointments—your therapist takes the time needed to deliver meaningful care and can customize the session to fit your needs.

Can I alternate between home and clinic sessions?

Yes. Some clients prefer to start at home and later transition to the clinic gym. Others alternate based on availability or treatment phase. We’re flexible—and design your plan around what’s best for you.

Do you treat conditions other than sports and orthopedic injuries?

Yes! While we’re known for our expertise in sports rehab and orthopedic care, our therapists are also experienced in treating:

  • Neurological conditions such as Parkinson’s disease, stroke recovery, neuropathy, and balance or gait dysfunction
  • Cardiopulmonary conditions, including post-COVID recovery, chronic fatigue, COPD, and cardiac deconditioning

If you’re unsure whether your condition is a good fit for physical therapy, we encourage you to reach out. We welcome complex or difficult cases, including those who may have had limited success with prior treatment. Our approach is highly individualized, and we’re committed to helping you find a better path forward.
